Our mission is to provide the best possible physical therapy services for our patients through skill-based care instead of time-based care.

To achieve this we are a one-on-one, skilled based model of reimbursement and are not currently an “in network” provider for any insurance plans. Decisions about treatment are a very personal experience and we believe that our success comes from understanding the goals of our clients. Physical Therapy fees*

In-Person Evaluation: $225

Virtual Patient Evaluation: $225

In-Person Treatment: $135

Virtual Patient Treatment: (50-60 Minutes)**: $135

*You have the right to receive a “Good Faith Estimate “ explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. A Good Faith Estimate can be provided to you in writing 24 hours prior to your medical service or items you receive by emailing at info@takecontrolphysicaltherapy.com.

**The actual amount of time spent will be determined by the physical therapist and is based on the physical therapist’s evaluation of the patient during the appointment. The fee for the service is the same regardless of how many minutes within the time range are used.

FAQ

 

CAN I USE MY HEALTH SAVINGS ACCOUNT FOR SERVICE?

Yes, For patients who have access to a Health Savings Account, we can and will accept payment for your treatment with a Health Savings Account credit or debit card.

 

WHAT STEPS ARE INVOLVED IN SUBMITTING A CLAIM TO MY INSURANCE COMPANY

The process is actually quite simple:  Take Control Physical Therapy will provide you with an invoice at the time of service, and you may submit that invoice and receipt to your insurance company for reimbursement. You may choose to submit bills following each visit, one time per month, or at any other interval, typically up to one year following your treatment visit.  

As a courtesy, we will provide you with an invoice that will include the applicable diagnosis and treatment codes as mentioned above.  The invoice has all of the necessary  information (business name and address, tax ID, national provider identification, etc.) as well as the patient’s ICD-10 (diagnosis) and CPT (billing) codes. You may choose to file the invoice with your insurance plan to see if any of the services are reimbursable under your particular plan.  Take Control PT does not guarantee that that any payment for Take Control PT physical therapy services will be reimbursed by your insurance plan or credited towards satisfying any deductible of your insurance plan.  

 

DO YOU HAVE A CANCELLATION POLICY?

Yes, You must provide Take Control PT with at least a 24-hour notice to cancel or reschedule your appointment or you will be charged the full amount of your scheduled appointment as a “no show” fee for missing the appointment.  All “no show” fees must be paid before a new appointment can be scheduled. Patients that repeatedly fail to provide the requisite notice prior to an appointment or pay their “no show” fees may be discharged from Take Control PT.
